These terms apply when you use the Birchgrove Clinic screening report service.

Who Provides The Service

The service is provided by Birchgrove Clinic Pty Ltd (ABN 44 696 521 874). Contact: info@birchgrove.health.

What The Service Includes

For the stated fee, Birchgrove Clinic provides an online adult ADHD, autism and mental health screening report service. The service may include:

  • a self-report questionnaire
  • optional uploaded documents
  • up to 2 optional support person questionnaires
  • a structured screening report generated from submitted information and issued by Birchgrove Clinic

Important Clinical Limits

The report is a screening report only. It is generated from the information submitted through the service and is intended to support reflection and discussion with a qualified health professional.

The service does not provide:

  • a diagnosis
  • a prescription or medication recommendation
  • a medical certificate
  • a treatment plan or clinical recommendations
  • a psychiatrist, psychologist or GP assessment
  • a workplace or disability capacity assessment
  • urgent mental health care or crisis support.

The report should not be used as a replacement for clinical assessment or advice from a qualified health professional.

Urgent Help

This service is not monitored as an emergency or crisis service. If you are at immediate risk or need urgent help, call 000. If you need urgent mental health support in Australia, contact Lifeline on 13 11 14 or your local mental health crisis service.

Eligibility

The service is designed for adults. By using the service, you confirm that you are at least 18 years old and that the information you provide is about you, unless we clearly allow another arrangement.

Your Responsibilities

You agree to:

  • provide accurate and complete information
  • use your own email address and keep sign-in links secure
  • upload only documents you have the right to provide and that are relevant to the report
  • obtain permission before giving us a supporting person's contact details
  • understand that incomplete, inaccurate or misleading information may affect the usefulness of the report.

Support Person Questionnaires

You may invite up to 2 supporting people to provide information. Support person questionnaires are optional. Birchgrove Clinic is not responsible if a supporting person does not respond, gives incomplete information or provides information you disagree with.

Supporting person information may be included in your screening report where received.

Document Uploads

Uploaded documents are optional supporting materials. Where provided and technically supported, they may be appended to your issued report as supplied. Their usefulness may be limited by document quality, relevance, readability, length, format or the information contained in the document.

Only upload documents you have the right to provide.

Payment And Refunds

Payment is required before the report service is provided and before an issued report can be downloaded. The current price is $45.00 AUD, collected through Stripe Checkout when you generate the PDF.

Because this is a digital report service, payments are generally not refundable once your report has been generated. If the service cannot be provided because of an issue on our side, contact us and we will review whether a replacement, correction or refund is appropriate.

Nothing in these terms limits rights you may have under the Australian Consumer Law, including consumer guarantees that cannot be excluded.

Availability

We aim to keep the service available, but availability may be affected by maintenance, technical issues, third-party services, security events or factors outside our control.

Use Of The Report

You may download, keep and share your report with health professionals or other people you choose. You are responsible for deciding who you share it with.

The report should be interpreted by a qualified health professional in the context of your full history and circumstances.

Intellectual Property

Birchgrove Clinic owns or licenses the website, templates, structure, wording, branding and report format. You may use your issued report for your own personal health purposes, but you must not copy, resell, reproduce or adapt the service, templates or report format for commercial use.

Privacy

Our Privacy Policy explains how personal information is collected, used, stored and shared for this service.

Liability

To the maximum extent permitted by law, Birchgrove Clinic is not responsible for loss arising from inaccurate or incomplete information you provide, your use of the report as a substitute for clinical care, third-party service interruptions, or decisions made by third parties after you share the report.

Nothing in these terms excludes, restricts or modifies any right or remedy that cannot be excluded under Australian law.

Changes To These Terms

We may update these terms from time to time. The current version will be published on this page. The terms displayed at the time you start a paid report generally apply to that report, except where a change is required by law or is needed to protect safety, security or service integrity.

Governing Law

These terms are governed by the laws of Queensland and the Commonwealth of Australia. The courts of Queensland have non-exclusive jurisdiction.
